Travel Immunisations

If you are travelling abroad, you may require certain travel vaccinations.  Our travel service is offered across both sites of the practice.

Please download the Travel Risk Assessment Form which can be found Travel Documents one form per person travelling.

You will need to return this form to the practice for the Practice Nurse to assess.  You will then be advised if an appointment is required.  Please call the practice 5 working days after you have returned the form, if you do not hear anything.

Travelling Last-Minute?

It is important to make this initial appointment as early as possible  - at least 6 weeks before you travel - a second appointment may be required with the practice nurse to actually receive the vaccinations.  Some vaccines have to be ordered as they are not routinely kept as a stock vaccine.  Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.
