New Patient Registration
You are welcome to register as a new patient with us if you live within the area we serve. You may also register as a temporary resident if you are staying in our area for a short period.
New patients wishing to register with the practice should ideally attend the surgery with their medical card showing their previous GP details and NHS number. Alternatively you may complete and submit the registration application online below (Please ensure sure you complete both the Registration form and the New patient questionnaire below).
If you do not wish to use the online registration facility, you may print off and complete both the Registration form and the Patient Questionnaire and bring both forms to the surgery. It would be helpful if as much information as requested on the form can be given as this will speed up the registration process. If you are unsure of your NHS number this can be obtained from your previous GP. Once the registration form has been returned/ submitted the details will be checked to ensure they are correct and that your address falls within the practice catchment area. You may also be required to provide of your residential address if necessaryie: a utility bill, bank statement etc. Part of the registration process involves a new patient check which will be offered to you at the time of registration.
You may complete the Registration Form along with the Medical Questionnaire and submit both online. Alternatively if you do not wish to submit these forms on line you may print off both copies and bring them after completion to the surgery if you wish to register with our practice.

To get access online, view your records, book appointment and request your prescriptions.
Fill out a form and bring into reception along with photo ID and a recent utility bill (last 3 month)
If you wish to register with Manor Practice online please ensure both the Registration and Medical Questionnaire forms are completed and submitted.
Please note the practice is not able to process submissions for blood or organ donation as per the registration form. If you wish to register you can do this seperately by either telephoning 0300 123 2323 or visiting the following websites http://www.uktransplant.org.uk/ or http://www.blood.co.uk/
Mobile Phones are a convenient, efficient and cost effective method for us to communicate with our Patients.
We will never include confidential information within the text.
If you have a mobile and are happy for us to communicate with you, please don't forget to include your mobile number when registering and complete the mobile consent form. If you do not have a mobile we will continue to write to you as normal.
We are encouraging all patients who have a mobile phone to enable text messaging.
